4/28/2023 EnchantaVaughn Approved (AM)Behavior: During morning therapy hours, client was reported on task and behaving well. Client displayed a cheerful demeanor as he greeted QMHP over the phone. Client was helpful to his peers who were having trouble hearing their teacher review important information by volunteering to speak up. Client was courteous to authority requests and demands following directives and paying attention to his assignments that were due by the end of class. The client was provided with positive feedback as he displayed a realistic understanding of his recurrent pattern of mental illness symptoms. Client obtained objectives 1, 2, and 3. (PM) QMHP acknowledged client effort to make things right without getting upset or causing a scene during his classes. QMHP prompted client to stay focused on the bigger picture and continue to work towards his virtual goals in order to complete his treatment objectives and to learn new virtual skill sets. QMHP provided client with prompts to initiate conversation and discuss any difficulties or struggles he may be experiencing during this time. QMHP praised client for not lashing out and choosing to be the bigger person in chaotic situations. QMHP helped client problem solve when faced with negativity or peer conflict. QMHP assisted client with being involved with his studies and after school activities.(EOD) Client presented a cooperative manner and reassured he would be on his best behavior. Client did not display any outbursts during the role play exercise and impulsive behaviors were at a minimum. Client had difficulty understanding some of his coursework, due to minor distractions but got right back on track. Client agreed to practice interventions in and out of school to show his strengths and virtual improvements. Client stated he learned how to be the bigger person. Client displayed sufficient progress with all of his activities.
